The evaluation done by our doctors involves a consultation, a review of your medical records, and a physical exam. In addition to performing the evaluation at your visit, we will also walk you through the process of uploading and submitting your forms to the state of Arizona Department of Health Services (ADHS) using their online application. You will need to bring your valid Arizona driver license or U.S. passport and a credit card to your visit to complete this step. You can expect to receive your card in the mail from ADHS in about one week.
